Does the 15% maintenance cost increase for the 4th maintenance category kicks in immediately once you have the 4th type of aircraft within your fleet, or only when you actually program it to fly? Considering the fact, that you incur maintenance fees/charges only when the aircraft flies, I am unsure about the idle 4th type causing increased cost or not. (I know if you offer it for sale at the market it does not count, but what about just being idle?)

As long as you have a 4th category aircraft in your Fleet you are able to use it even if it is only on the ground. Therefore you have to pay the increased costs for maintanance. And you are correct that if you offer a 4th category Aircraft for sale then there will be no costs.
At which point of time does the additional category increase costs - each flight after the 4th aircraft comes into the fleet until it's out or week closing or something else?
the moment you receive an aircraft that does not belong to the first three maintenance categories you already own, the costs for maintenence increace by 15%. maintenance is being calculated once an aircraft takes off from any given airport (if the aircraft had a maintenance break = was standing still for more than 2 hours), so the additional 15% will be charged beginning with the first aircraft to receive maintenance after the above mentioned aircraft joins your fleet.

IMO this needs to be changed. Imagine an airline (who also acts as a lessor) and you receive an aircraft back after a lease that does not belong to my categories and the costs for maintenance increases by 15%.

I agree... the 4th (and 5th etc. maintenance category should only apply once that category is scheduled into service and schedule activated.
Well, I guess no one is really into the current system as it doesn't make much sense and can be massively expensive. However, I think there is currently not much development priority behind this.
(For what it is, I take it as an additional risk for larger companies to limit growth. ^^)
