No demand short island hops

I have just started playing and have a couple of routes where I have zero passengers for a number of days. One route is between airports with 4 and 6 demand and no competition. The common thread is they are very close but on different islands. Is this something to do with Ground Networks.

the ground network doesn’t understand islands in its current state, so it will assume connectivity regardless of mountains or body of water inbetween.

there might simply not be any demand between those airports, maybe you don’t have traffic rights, maybe you have a negative ORS rating for your flights (pax only book positively rated flights).
please share your airline, server and some of the routes so we can have a look. it’s impossible to give more than a generic answer without having a look.

Airline “Sheila Flight” to and from Nassau : North Eleuthera, San Andros, South Andros

For Sheila Flight - All 3 airports listed from Nassau are only 1 bar, there will be minimal pax demand, if any, without a wave system so pax can connect, which you don’t seem to have. Also you have a competing airline at Nassau called Song Island Air. They use EJets exclusively while you have ATR72. Jets are more highly rated than prop aircraft, so his flights have a better rating. Therefore any routes where you compete, his flights will fill before yours all other factors being equal. So that may be another reason you are struggling to fill your flights.

For Sheila TT (You have do have passengers rights as Trinidad & Tobago has unrestricted market access) it may be more down to having far too many daily flights (6 daily flights!). Again without possible connections for pax, you will never fill at ATR72 flying 6 times a day between a 5 bar and 4 bar airport.
